What are the Requirements?

- Full name and address of the complaint
- Full name and address of the person complained of, as well as his position in the Department of Education
- A narration of the relevant and material facts which shows the act or omissions as allegedly committed by the person
- Certification of Non-Forum Shopping
- Complaint shall be under oath
